I had also been through a long-distance relationship and it is really hard to keep the relationship strong. Eventually, we broke up, not because we do not love each other anymore but the set-up did not work anymore. We found less time to communicate with each other, and the absence of intimacy made it even harder. In the end, we decided to let go of each other. I am not saying you follow our example. Two years is too long to just throw away. But considering you are constantly fighting with each other, maybe it is time to re-evaluate everything about your relationship. Love is sometimes just not enough... As with your insecurities, it is understandable since he has cheated you many times before. Try giving more trust, or find if there are reasons for your insecurities. When you find out, may be that would be the time to take a big decision.

Sounds to me that you've got more emotionally invested into this relationship than he does.  You invest much of yourself into those words, and maybe to him they are merely words.  Without the inflection of facial gestures, warm acceptance or cold disapproval, and touch, it never lasts too long.  Being mystified by that suspense and wonderment and amplified expectations only lasts so long, for both parties involved.
